In 2025, we doubled down on our mission with bold action, measurable impact, and partnerships that set a new standard for integrated care. Patients, providers, and practices are taking notice and joining our mission to reimagine behavioral health.

Here’s how evolvedMD delivered on its promise:

1. More Clinics, More Clinicians, More Lives Changed

The market is responding with enthusiasm to our innovative collaborative care model (CoCM). This year, we expanded behavioral health services into more primary care practices and entered new specialties like Diabetes and Dementia. Here’s what we saw:

  • Unique Patients: 42,000, a 13.5% increase from 2024.
  • New Patients: 29,750, a 10.2% increase from 2024.
  • New Clinics: 79 new clinics implemented, totaling 250+ sites across the nation.
  • New Employees: 41 new full-time employees spanning clinical, admin, and leadership.

2. Measuring Success, One Patient at a Time

We can’t radically change healthcare without measuring outcomes. Using screening tools like the PHQ-9 and GAD-7, we track each patient’s baseline and their progress over time. This approach helps us adjust care as needed, graduate patients faster, and create capacity for others seeking help. Over the past 12 months, our clinicians have delivered over 122,000 patient-facing hours, achieving:

  • Depression (PHQ-9) and Anxiety (GAD-7): 64% of patients achieved a 50% reduction or 10-point reduction from their most recent max assessment score within five months.

 

Why is this important? Patients in our collaborative care program improve much faster compared to traditional care and competing integrated care programs. A win-win for both patients and providers alike, our measurement-based approach delivers best-in-class results.

 

3. Third Party Validation Confirms a New Gold Standard for Integrated Care

A leading healthcare actuarial consulting firm independently evaluated our innovative model, analyzing 1.3 million patients over two years. Their findings show because of evolvedMD, practice partners see:

  • 15% lower risk-adjusted total cost of care.
  • 15% fewer psychiatric professional visits.
  • 3-4x more PCP referrals for onsite CoCM compared to virtual.
  • 17% stronger patient-PCP relationships.

5. Five Years of Partnership, Thousands of Lives Changed with HonorHealth

You’ve seen our broader impact, now see what happens when we partner with just one primary care system: HonorHealth.

Our impact since 2020:

  • Patients Served – Nearly 17,000 patients served.
  • Patient Outcomes – PHQ-9 response rate: 64%; GAD-7 response rate: 63%.
  • Clinic Partnership – 27 clinics enriched with mental health services, with more to come.

 

Why is this important? Our transformational partnership began with a pilot at just four HonorHealth Medical Group primary care sites in 2020. Scaling to all 27 sites in the years since, we removed barriers to mental health care and delivered timely, evidenced-based support.

8. Closing the Gap to Comprehensive Psychiatric Med Management

Grounded in evidence-based care and supported by evolvedMD, we launched evolvedBH to connect patients with licensed, qualified practitioners for psychiatric services and medication management. Led by Dr. John Brewer, Chief Clinical Officer of Psychiatry, evolvedBH manages more complex behavioral health needs and gets patients impactful care, faster.

Why is this important? With evolvedBH, patients can have their first appointment within five days of referral—not months.
