Last week, we launched a monthlong campaign to highlight our amazing staff at evolvedMD, starting with the Ongoing Support team.
This week, it’s our pleasure to introduce our Care Coordination team. Unique to evolvedMD’s integrated healthcare model, Care Coordinators maintain close communication with our Behavioral Health Managers, conduct phone-based outreach to patients, and provide patients with community resources relating to housing, food insecurity and other social determinants of health. Offering external support services and compassionate patient advocacy, our Care Coordinators are key members of the Behavioral Health Team and we couldn't do what we do without their dedication and hard work!
About Ashley: Ashley is a compassionate behavioral health professional who has made it her mission to increase access to quality mental health services so people can live healthier, more fulfilling lives. With a Bachelor’s in Addictions Counseling from Grand Canyon University, a Master’s in Social Work from Northern Arizona University, and experience working at a residential facility for patients with substance use disorders and severe mental health conditions, she has been a critical member of evolvedMD’s Care Coordination since joining in 2018. When she’s not working, you’ll find her dancing, playing with her two dogs, and passionately serving as an advocate for mental health awareness in her community.
Fun Fact: She loves dancing! From age 3-23, she did Ballet, Tap, Jazz, Pointe, Modern, Lyrical, and the Silks/Trapeze at a dance studio in Phoenix.
About Humberto: Humberto is a dedicated Care Coordinator who strives to make a positive, long-lasting impact on each patient he interacts with. He has over 7 years of experience in social work and has enjoyed gaining hands-on experience alongside evolvedMD’s Behavioral Health Managers. Originally from Nogales Sonora, Mexico, he has lived in Arizona since 2010 and graduated from Arizona State University with a Bachelor’s in Criminal Justice and Sociology. In his free time, you’ll find him sweating it out at CrossFit, hanging out with his dogs, and advocating for social justice across a wide variety of causes.
Fun Fact: He has double nationality in both Mexico and the USA.
About Janet: Janet has devoted her whole life to being a positive influence and making sure the people around her feel heard, which makes her a tremendous asset on the Care Coordination team. With a degree in Psychology from Arizona State University and over 7 years of professional experience, she has worked with patients in outpatient behavioral health clinics, counseling agencies, and non-profit organizations. Outside of work, she loves spending time with her 3-year-old, relaxing with a good book, and dancing it out for self-care.
Fun Fact: She loves exploring new cities and exciting places, and travels to a new country once a year. Her next trip is to the Dominican Republic!
